Form Creator
Turn any PDF into a fillable form by drawing text boxes, checkboxes, dropdowns and signature fields.
About this tool
Form Creator adds interactive AcroForm fields to an existing PDF. Pick a field type — single-line or multi-line text, checkbox, radio button, dropdown, list, signature or button — and drag on the page where it should go. A single click places a field at a sensible default size.
Select a field to set its name, default value, options and whether it is required. Radio buttons that share a group name work together; duplicate a field to build rows quickly.
The result works in Adobe Acrobat, browsers and mobile PDF apps, and can be filled with Mofumi's Form Filler. Everything runs locally.
